ServiceLink at Community Partners is part of the Aging and Disability Resource Centers / ServiceLink network across the state of New Hampshire.  We follow a No Wrong Door model, which means you can rely on us as your single access point to inquire about and obtain community long term supports and services.

You can expect to receive courteous customer service, objective and unbiased information on the full range of long term care supports and services and other available community resources.  Staff will help to link you with needed services and provide person-centered one-on-one assistance and decision support.
