Supports and Services are available to all residents of Strafford County, New Hampshire who meet specific eligibility requirements. Community Partners provides services to residents of Barrington, Dover, Durham, Farmington, Lee, Madbury, Middleton, Milton, Milton Mills, New Durham, Rochester, Rollinsford, Somersworth, and Stafford.
Family-Centered Early Supports and Services are available to families with infants and toddlers who demonstrate a 33% delay in an area of development, have a diagnosis or a condition which is known to have an impact on development, or have a history of child and family factors which are likely to impact the child's development. At the age of three, if a child's developmental delay represents a disability, eligibility for services from Community Partners may continue (see the next section).

Over the age of three, an individual may be eligible if there is a documented disability that is attributable to at least one of the following:
Mental Retardation
Cerebral Palsy
Epilepsy
Autism
Specific Learning Disability
Any other condition of an individual found to be closely related to mental retardation as it refers to general intellectual functioning or impairment in adaptive behavior or to require treatment similar to that required for individuals with mental retardation.
In addition:
The disability must have occurred in the developmental period (before age 22)
The disability has continued, or can be expected to continue indefinitely,
The disability constitutes a severe handicap to the individual's ability to function normally in society.

supports and services (ages 3 and above) may be made to the Intake
Coordinator by calling (603) 516-9300.
In the event that an individual is suspected of meeting eligibility requirements but sufficient documentation does not exist, the area agency may extend conditional eligibility for a specified period of time until documentation is obtained which establishes eligibility or ineligibility.
"Acquired Brain Disorder" means a disorder of the brain or nervous system that is not congenital, presents a severe and lifelong disability condition which significantly impairs an individual's ability to function in society, occurs after the age of 22 but prior to age 60, and is due to one or more of the following:
Physical Trauma as a result of a car, motorcycle, or bicycle accident, or other related physical accident or occurrence;
Infectious disorder such as encephalitis or meningitis;
Cerebral vascular disease such as stroke;
Demyelinating or other inflammatory diseases such as multiple sclerosis;
Toxic metabolic disorder such as anoxia; or
Other related neurological disorders such as Huntington's Disease
