Eligibility for Services

Supports and Services are available to all residents of Strafford County, New Hampshire who meet specific eligibility requirements. Community Partners provides services to residents of Barrington, Dover, Durham, Farmington, Lee, Madbury, Middleton, Milton, Milton Mills, New Durham, Rochester, Rollinsford, Somersworth, and Stafford.

Birth through Age 3

Family-Centered Early Supports and Services are available to families with infants and toddlers who demonstrate a 33% delay in an area of development, have a diagnosis or a condition which is known to have an impact on development, or have a history of child and family factors which are likely to impact the child's development. At the age of three, if a child's developmental delay represents a disability, eligibility for services from Community Partners may continue (see the next section).

Age 3 and above

Over the age of three, an individual may be eligible if there is a documented disability that is attributable to at least one of the following:

Conditional Eligibility

In the event that an individual is suspected of meeting eligibility requirements but sufficient documentation does not exist, the area agency may extend conditional eligibility for a specified period of time until documentation is obtained which establishes eligibility or ineligibility.

 

Acquired Brain Disorder (Age 22 - 60)
ABD Waiver HeM 522.02

"Acquired Brain Disorder" means a disorder of the brain or nervous system that is not congenital, presents a severe and lifelong disability condition which significantly impairs an individual's ability to function in society, occurs after the age of 22 but prior to age 60, and is due to one or more of the following:
