is seeking talented individuals to provide services to clients in the home, the
community, and our offices. Positions are available in the following areas:
COMBINED BEHAVIORAL HEALTH AND DEVELOPMENTAL SERVICES POSITIONS
JOB DEVELOPER: Will work with local businesses to evaluate job opportunities for our consumers. Job duties include working with consumers seeking vocational placement by providing job coaching and career planning activities, designing and editing resumes and monitoring training supports. Also, maintain employer relations, facilitate job matches and provide employer supports. Qualifications: Bachelor’s Degree in Human Services or related field required. Ability to assess placement, provide support to consumers and provide clear communication with prospective employers required. Familiarity with people with a developmental disability and/or mental illness preferred.
DEVELOPMENTAL SERVICES POSITIONS
DAILY LIVING SKILLS INSTRUCTOR: Description: Assists consumers of
Residential Services to develop daily living skills while facilitating and
promoting the skills necessary for increased independence. Works directly with
the consumers in their homes and in the community, to provide a quality daily
living environment in accordance with NH State certification standards.
Qualifications: High school diploma or equivalency required. One year
residential service experience preferred.
COMMUNITY SUPPORT STAFF: Description: Responsible for providing consumers
living independently with opportunities to develop skills and and to access
trainings, education and resources to enable them to maintain their independence
and to assist those consumers living at home to make the transition to living on
their own in the community. Qualifications: AS degree related to Human Services
and two years experience working with adults with developmental disabilities
preferred.
ASSISTANT PROGRAM COORDINATOR: Description: Responsible for overseeing
the Day Program site in the absence of the Program Coordinator. Assists
Coordinator with day-to-day operation of service. Ensures adequate daily
staffing ratios and works with supervisor to resolve staffing issue priorities.
Prepares and participates in assigned consumers ISPs. Works with Supervisor to
develop and implement plans to facilitate accomplishment of assigned consumers’
ISP objectives. Works directly with consumers in their daily activities as
needed and acts as liaison between consumers and community members. Maintain
current daily schedules for clients and make changes to schedules as needed.
Qualifications: High School diploma or equivalent required. Experience in the
Human Service industry including supervisory experience preferred. Must become
medication certified according to HeM 1201s and ensure medications are
administered in accordance with HEM 1201. Willingness to learn is essential.
Must be able to assist consumers in and out of vehicles.
BEHAVIORAL HEALTH SERVICES POSITIONS
MEDICAL RECORDS COORDINATOR: Description: Provide daily oversight of the medical records function at all Behavioral Health sites. Perform duties of Medical Records Assistant. Create and manage data base for managing medical records and establish systems for purging, archiving, or destroying records. Conduct periodic audits of charts for compliance with state and federal mandates and ensure readiness of charts for state compliance audits. Maintain tracking system for medical records and maintain form files. Qualifications: High school diploma and at least three years of experience successfully handling multiple priorities in a professional environment required. Must be detail oriented, self starter, able to work independently and maintain confidentiality on all matters including consumer/client information, employee information and correspondence. Full time, hourly position. Schedule may vary according to needs of agency and may include nights and weekends.
ARNP: Description: Full Time Psychiatric Nurse Practitioner needed to work with our psychiatric team in meeting the behavioral health needs of people in our region. As a member of our team, this position will be responsible for developing and managing a collaborative care model addressing the mental health and physical health of people in partnership with our local Community Health Clinic. Qualifications: Master’s degree in Social Work, Counseling, Marriage & Family Therapy or related field. Current NH ARNP License, NH RN License, current DEA required. Must maintain ANCC certification as Clinical Nurse Specialist. One year experience as Nurse Practitioner in a mental health care setting preferred.
ADMISSIONS SPECIALIST: Description: Responsible for the BHS centralized
intake and admissions system to ensure easy access to services. Receive all
initial requests for service, collect and record necessary demographic and
insurance information, schedule appointments and track requests for services
agency-wide. Provide follow-up to all requests for service and maintain working
knowledge of insurance carriers, including Medicaid, Medicare and commercial
insurance carriers. Qualifications: Bachelor’s degree in Social Work, Psychology
or related field required. Ability to work in a fast paced environment. Must
possess strong organizational and communication skills and attention to detail.
Must be able to exercise independent judgment and use discretion in the
performance of these duties while operating within the limits of standard
practice. Full time, 40 hr/wk, hourly position.
